Once you've made your appointment, we will send you a secure link to complete the necessary forms prior to your telehealth or in-person visit. It's that easy!

To get the most out of your appointment, it's important to jot down the reason for your visit and any symptoms you're experiencing before you meet with a Lakeside provider. We recommend having the following on hand:

The Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule affords members the right to receive a notice that describes how health information may be used and disclosed and how to get access to this information.
