Vital Care Services

According to the Centers for Disease Control, chronic diseases are responsible for 7 out of 10 deaths each year, and treating people with chronic diseases accounts for 86 percent of our nation’s healthcare costs.

Through our Vital Care program, we partner with you to help your high-risk patients navigate the healthcare system by providing:

  • Education/counseling
  • Urgent referrals
  • Set up of home/social service or placement when needed
  • Vital Care case managers who become your patient’s best ally

Case management services offered to patients include:

  • Transitions of care – followed closely for 30 days post-discharge
  • Authorization processes and DME
  • Establishment with primary care physician/referrals to specialists
  • Medication reconciliation by pharmacists
  • Referrals to social workers
  • Identification of the need for home visiting providers
  • Information about the Rite Aid Pharmacy educational program
  • Assistance with OP blood transfusions
  • Direct admin or transfer to skilled nursing facility
  • Assistance with referrals to mental health
  • Information about local urgent care centers
  • Engagement of palliative (supportive) care
  • Home IV infusion for hydration or antibiotics
  • Arrangement of specialty services and community services (CBAS, IHSS, ADHS)

What are the patient criteria for the Vital Care program?

Any senior or commercial patient who is at risk for multiple admissions/emergency room visits, specifically:

  • Two chronic medical conditions + one high risk criteria (poor social support, functional status, psychological condition, substance abuse, and multiple admissions)
  • One poorly-controlled chronic medical condition + two high risk criteria
  • WHEN IN DOUBT, refer your patient (submit an authorization)
