As a benefit to the patients at the Lakeside Community Healthcare (“Lakeside”) practice site located at this location (the “Office”), Lakeside will provide free wireless access for visitors to use with their own personal notebooks, laptops, tablets and other mobile devices (“Personal Devices” or individually a “Personal Device”). These access points are unsecured and accessible only during normal business hours. Office staff can provide general information or handouts for connecting your device to the wireless network, but cannot troubleshoot problems related to your Personal Device or assist in making changes to your Personal Device's network settings and/or hardware configuration. The Office cannot guarantee that your Personal Device will work with the Office’s wireless access points.

TERMS OF USE:-

All wireless access users should have up-to-date virus and malware protection on their Personal Devices.

All users are expected to use the Office’s wireless access in a legal and responsible manner, consistent with the educational and informational purposes for which it is provided. Use of the Office wireless network is entirely at the risk of the user. The Office and Lakeside disclaim all liability, and the member holds the Office and Lakeside harmless, for loss of any information or data however stored (e.g. credit card numbers, protected health information, passwords, financial information, photos, emails, etc.) or damages, direct or indirect, resulting from that loss. Moreover, the Office and Lakeside disclaim and the member holds the Office and Lakeside harmless for all liability for damages, direct and indirect, including those caused to your hardware or software on the Personal Device, due to electric surges, security issues or consequences caused by viruses or hacking along with but not limited to damage to other Personal Devices, computers, systems, networks, etc., caused by any virus, malware, or other form of malicious code transferred to the Personal Device while using the Office’s wireless network or any other damages of any kind flowing from the use of the Office’s wireless network.

The Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule affords members the right to receive a notice that describes how health information may be used and disclosed and how to get access to this information. Lakeside Community Healthcare Medical Group is required to send a notice ("Notice of Privacy Practices") to members of our insured Health and Long Term Care plans.
